Approving adverse action against the Massage Center-B (Home Location) and Massage Practitioner licenses held by Laurie S. Reis, d/b/a Massage For A Lifetime 1429 Van Buren Avenue.

 

Body

WHEREAS, the Massage Center-B (Home Location) and Massage Practitioner licenses held by Laurie S. Reis d/b/a Massage For A Lifetime (License ID #20030004922) for the premises located at 1429 Van Buren Avenue in Saint Paul was the subject of a Notice of Intent to Suspend Licenses dated November 2, 2016, stating the licensee failed to pay delinquent license and late fees; and  

 

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit information required to maintain the licenses; and  

 

WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to pay the delinquent license and late fees and submit the required information or request a public or administrative hearing; and 

 

WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information or request a public or administrative hearing by November 14, 2016, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the Massage Center-B (Home Location) and Massage Practitioner licenses held by Laurie S. Reis d/b/a Massage For A Lifetime are hereby suspended for failure to pay delinquent license and late fees.

 

FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit information required to maintain the license.

 

FINALLY RESOLVED, the license will remain suspended until the delinquent license and late fees are paid in full and all required information is submitted to the Department of Safety and Inspections.
