Approving adverse action against the Cigarette/Tobacco license application submitted by Mohammed Sardar Khan, d/b/a Grand Hookah at 688 Snelling Avenue North.
Body
WHEREAS, the Cigarette/Tobacco license application submitted by Mohammed Sardar Khan d/b/a Grand Hookah (License ID #20130003057) for the premises located at 688 Snelling Avenue North in Saint Paul was the subject of a Notice of Intent to Deny License dated August 21, 2013, stating the license applicant failed to submit information to DSI required to complete the application process; and
WHEREAS, the licensing office recommended a denial of the license application; and
WHEREAS, licensee did respond to the Notice of Intent to Deny License and speak with a DSI inspector, who gave the licensee another copy of the Notice and a list of documents he needed to complete and return; and
WHEREAS, the licensee then sent an electronic mail to DSI inquiring why additional information was needed for his pending application; and
WHEREAS, after that electronic communication, the licensee failed to follow-up with DSI to schedule the Zoning Inspection, submit the required information to complete the application process, request a public or administrative hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to schedule the Zoning Inspection, submit the required information, request a public or administrative hearing or withdraw the license application by September 2, 2013, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Cigarette/Tobacco license application submitted by Mohammed Sardar Khan d/b/a Grand Hookah is hereby denied.
