WHEREAS, adverse action was taken against the House Sewer Contractor license application submitted by Dean's Professional Plumbing d/b/a Dean's Professional Plumbing (License ID #20100005226) for the City of Saint Paul by Notice of Intent to Deny License dated September 19, 2011, alleging licensee failed to submit a current original Uniform, License and Permit Bond in the amount of $10,000.00; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny License to submit the required bond or request a hearing; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit the required bond or request a hearing by September 29, 2011, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the House Sewer Contractor license application submitted by Dean's Professional Plumbing d/b/a Dean's Professional Plumbing is hereby denied.
