Approving adverse action against all licenses held by Saigon, Inc. d/b/a Saigon located at 704 University Avenue West in Saint Paul.

WHEREAS, adverse action was taken against all licenses held by Saigon, Inc., d/b/a Saigon (License ID #20080004642) for the premises located at 704 University Avenue West in Saint Paul by Notice of Intent to Suspend Licenses dated November 8, 2010, alleging licensee failed to submit a completed Certificate of Compliance Minnesota Workers Compensation Law form; and
WHEREAS, per Saint Paul Legislative Code ยง 310.05(m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit information required to maintain the license; and
WHEREAS, licensee did not respond to the Notice of Intent to Suspend Licenses to submit the required information or request a hearing; and
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to submit the required information or request a hearing by November 18, 2010, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that all licenses held by Saigon, Inc., d/b/a Saigon are hereby immediately suspended and the licensee is order to pay a $500.00 matrix penalty for failure to submit a completed Certificate of Compliance Minnesota Workers Compensation Law form. Payment of such penalty shall be made within thirty days of the date of the adoption of this resolution.
