Authorizing the Department of Public Works to Enter into a Contract with JotForm Services for the Lease of their Interactive Form and Data Collection Application.

Authorizing the Department of Public Works to enter into a contract with JotForm Services (“JotForm”) for lease of their interactive form and data collection application.

 

WHEREAS, the City of Saint Paul (“the City”) entered into a Residential Solid Waste, Yard Waste, and Bulky Waste Collection Agreement (“the Agreement”) with the St. Paul Hauler’s (“the Consortium”) for the collection of residential trash; and

 

WHEREAS, the Agreement requires the Consortium to report data on annual tonnage of garbage and yard waste, number of active temporary service holds, and number of bulky items collected in the program; and

 

WHEREAS, the Agreement requires the City to provide the means for the Consortium to submit this data; and

 

WHEREAS, the City explored several vendors for this service; and

 

WHEREAS, the City had identified JotForm as the best vendor for the City to provide to the Consortium to submit this data; and

 

WHEREAS, the JotForm program is a major component for the Department of Public Works to manage garbage, bulky waste and yard waste collection data within the City, and will increase efficiency, security, and service for the transfer of records from the Consortium; and

 

WHEREAS, JotForm enrollment requires a “click-through” acceptance of standard terms and conditions, which the company will not alter; and

 

WHEREAS, the agreement includes an indemnification clause, which clause the Council finds reasonable and necessary to transition to more reliable Consortium reporting software; and

WHEREAS, the annual JotForm subscription fee of $348 is covered under the existing Public Works budget; now, therefore, be it

RESOLVED, that the Department of Public Works is authorized to accept the Terms of Use for and enter a contract with JotForm.
