Approving adverse action against the Massage Practitioner License held by Mikayla Eldredge (License ID #20180003284).
Body
WHEREAS, the Massage Practitioner license held by Mikayla Eldredge (License ID #20180003284) for the City of Saint Paul were the subject of a Notice of Violation Recommendation for imposition of $500 Matrix Penalty dated March 30, 2020; and
WHEREAS, the Licensee failed to provide a revised Certificate of Insurance to show that the Licensee had active General and Professional Liability Insurance from December 18, 2019 to December 31, 2019 in violation of Saint Paul Legislative Code § 414.03 (c); and
WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office recommended a $500.00 matrix penalty; and
WHEREAS, the licensee responded to the Notice of Violation on April 5, 2020 and admitted the violation but requested a public hearing to contest the penalty via Electronic Delivery Mail and
WHEREAS, a public hearing was held August 5, 2020 and now, therefore, be it
RESOLVED, Mikayla Eldredge (License ID #20180003284) is hereby ordered to pay a matrix penalty of $500.00 for failure to provide proof of active General and Professional Liability Insurance starting December 18, 2019.
Payment of such penalty shall be made within thirty (30) days of the date of the adoption of this resolution.
