Body
WHEREAS, on July 17, 2019, YTS Companies, LLC d/b/a Yetzer’s Tree Service (License ID# 20130003380) submitted payment for license renewal of the Tree Trimmer & 1 Vehicle and Tree Trimmer licenses; and
WHEREAS, on October 8, 2019, the Department of Safety and Inspections (“Department”) notified Yetzer’s Tree Service that their licenses could not be renewed until a copy of the current DOT inspection forms and General Liability Certificate with coverage dates of 5/1/2019 to 5/1/2020 were submitted to the Department; and
WHEREAS, on January 15, 2020, the Department sent Licensee notice that they were unable to complete their license renewal because the Licensee had failed to provide all of the information necessary to complete the renewal process; and
WHEREAS, the January 15, 2020 notice stated that if the licensee failed to obtain inspection approval for the vehicles to be used in the operation of the business, provide a Certification of Insurance showing evidence of insurance issued by an insurance company authorized to do business in the State of Minnesota, request a public hearing, or withdraw the license application by March 18, 2020, that the matter would be placed on the City Council consent agenda to impose the recommended penalty; and
WHEREAS, the Licensee did not respond to the notice by submitting proof that it had obtained inspection approval for the vehicles to be used in the operation of the business, provide a Certification of Insurance showing evidence of insurance issued by an insurance company authorized to do business in the State of Minnesota, request a public hearing, or withdraw the license application; now therefore, be it
RESOLVED, the Tree Trimmer & 1 Vehicle application submitted by YTS Companies, LLC d/b/a Yetzer’s Tree Service (License ID# 20130003380) for the City of Saint Paul is hereby denied.
