Approving adverse action against the Tree Trimmer & 1 Vehicle and Tree Trimmer - Each Additional Vehicle licenses held by Ivan’s Tree Service, LLC.

WHEREAS, the Tree Trimmer & 1 Vehicle and Tree Trimmer - Each Additional Vehicle licenses held by Ivan’s Tree Service, LLC d/b/a Ivan’s Tree Service, LLC (License ID #20170000662) for the City of Saint Paul were the subject of a Notice of Intent to Suspend Licenses dated September 12, 2018; and
WHEREAS, the Notice stated the licensee failed to provide evidence of insurance, obtain inspection approvals and submit current Minnesota Department of Transportation (MN DOT) inspection reports for two (2) vehicles licensed; and
WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit information required to maintain the licenses; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to provide evidence of insurance, obtain inspection approvals and submit current MN DOT inspection reports for two (2) vehicles licensed, request a public hearing or cancel the licenses; and
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to provide evidence of insurance, obtain inspection approvals and submit current MN DOT inspection reports for two (2) vehicles licensed, request a public hearing or cancel the licenses by September 24, 2018 that the matter would be placed on the October 10, 2018 City Council consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Tree Trimmer & 1 Vehicle and Tree Trimmer - Each Additional Vehicle licenses held by Ivan’s Tree Service, LLC d/b/a Ivan’s Tree Service, LLC (License ID #20170000662) for the City of Saint Paul are hereby suspended.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to provide evidence of insurance, obtain inspection approvals and subm...
