Approving adverse action against the Auto Repair Garage license held by Firestone Complete Auto Care at 491 Jackson Street.

 

Body

WHEREAS, the Auto Repair Garage license held by Firestone Complete Auto Care d/b/a Firestone Complete Auto Care (License ID #0015972) for the premises located at 491 Jackson Street in Saint Paul was the subject of a Notice of Intent to Suspend License dated March 1, 2018, stating the licensee failed to pay delinquent license fees, alarm permit fees and late fees; and

 

WHEREAS, the licensee was also required to submit a completed Certificate of Compliance Minnesota Workers’ Compensation Law form along with the license, alarm permit and late fee payments; and   

 

WHEREAS, the licensee responded to the Notice of Intent to Suspend License on March 8, 2018, to submit a completed Certificate of Compliance Minnesota Workers’ Compensation Law form but failed to include the delinquent license, alarm permit and late fee payments; and

 

WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to pay the delinquent license, alarm permit and late fees, request a public hearing or cancel the license by March 12, 2018, that the matter would be placed on the next available consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the Auto Repair Garage license held by Firestone Complete Auto Care d/b/a Firestone Complete Auto Care (License ID #0015972) for the premises located at 491 Jackson Street in Saint Paul are hereby suspended.   

 

FURTHER RESOLVED, the license will remain suspended until all delinquent license, alarm permit and late fees are paid in full to the Department of Safety and Inspections.
