WHEREAS, the Massage Practitioner license application submitted by Kevin P. Klein (License ID #20170003835) for the premises located at 1053 Grand Avenue Suite #113 in Saint Paul was the subject of a Notice of Intent to Deny License dated March 9, 2018; and
WHEREAS, on February 1, 2018, the applicant was notified by mail that the license application was still pending because he has failed to submit the following information: 1) either proof of the ability to have been licensed as a Massage Practitioner in Saint Paul based on completion of a national certification exam or 2) proof of successful completion of a postsecondary course of study that includes 500 contact hours at an accredited or license school; and
WHEREAS, the applicant did not respond to the Notice of Intent to Deny License to submit the required education information, request a public hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit required education information, request a public hearing or withdraw the license application by March 19, 2018, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license application submitted by Kevin P. Klein (License ID# 20170003835) for the premises located at 1053 Grand Avenue, Suite #113 is hereby denied.
