WHEREAS, the Cigarette/Tobacco license held by Elisea’s Snack Shop, LLC d/b/a Elisea’s Snack Shop (License ID #20110004624) for the premises located at 1885 University Avenue West, Ste. 100 in Saint Paul, was the subject of a Notice of Intent to Suspend License dated January 17, 2018, stating the licensee failed to pay delinquent license and late fees and submit a required Certificate of Compliance Minnesota Workers’ Compensation Law form; and

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information to maintain the license; and

WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees and submit the required information, request a public hearing or cancel the license; and

WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information, request a public hearing or cancel the license by January 29, 2018, that the matter would be placed on the next available consent agenda to impose the recommended penalty; now, therefore, be it

RESOLVED, the Cigarette/Tobacco license held by Elisea’s Snack Shop, LLC d/b/a Elisea’s Snack Shop (License ID #20110004624) for the premises located at 1885 University Avenue West, Ste. 100 is hereby suspended.

FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.

FINALLY RESOLVED, the license will remain suspended until all delinquent license and late fees are paid in full and required information is submitted to the Department of Safety and Inspections.
