WHEREAS, the Massage Practitioner license application submitted by Tracy Ann Porter (License ID# 20150002986) for the premises located at 2140 Ford Parkway in Saint Paul was the subject of a Notice of Intent to Deny License dated August 29, 2017; and
WHEREAS, the Notice stated the licensee failed to complete the requirements necessary to complete the application process; and
WHEREAS, as a result, the licensing office recommended license denial; and
WHEREAS, the City Attorney’s Office received a call on August 30, 2017, from Massage Envy staff stating the applicant was not working at that location; and
WHEREAS, the applicant did not respond to the Notice of Intent to Deny License to request a public hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to request a public hearing or withdraw the license application by September 8, 2017, that the matter would be placed on the next available consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license application submitted by Tracy Ann Porter (License ID# 20150002986) for the premises located at 2140 Ford Parkway in Saint Paul is hereby denied.
