Approving adverse action against the Second Hand Dealer license held by Ana Rosario, d/b/a Lulu’s Store at 776 - 7th Street East. (To be withdrawn)

 

Body

WHEREAS, the Second Hand Dealer license held by Ana Rosario d/b/a Lulu’s Store (License ID# 20160001098) for the premises located at 776 - 7th Street East in Saint Paul was the subject of a Notice of Intent to Suspend License dated June 13, 2017; stating the licensee failed to pay delinquent license and late fees and submit 1) location information of where business records will be kept and/or maintained; 2) proposed hours of operation and 3) a completed Certificate of Compliance Minnesota Workers’ Compensation Law form; and

 

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information to maintain the license; and  

 

WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees and submit the required information; and

 

WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information, request a public hearing or cancel the license by June 23, 2017, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the Second Hand Dealer license held by Ana Rosario d/b/a Lulu’s Store for the premises located at 776 - 7th Street East is hereby suspended for failure to pay delinquent license and late fees.

 

FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.

 

FINALLY RESOLVED, the license will remain suspended until all license and late fees are paid in full and the required information is submitted to the Department of Safety and Inspections.
