WHEREAS, the Massage Practitioner license application submitted by Birhanu Astaw Desta (License ID# 20160003864) for the premises located at 2140 Ford Parkway in Saint Paul was the subject of a Notice of Intent to Deny License (“Notice”) dated May 2, 2017; and
WHEREAS, the Notice stated that the license application had not been approved because the applicant failed to submit either proof of certification or proof of successful completion of the Massage Practitioner Examination; and
WHEREAS, the applicant did not respond to the Notice of Intent to Deny License to submit the required information to complete the application process, request a public hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny Licenses stated that if the applicant failed to submit the required information to complete the application process, request a public hearing or withdraw the license application by May 12, 2017, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license application submitted by Birhanu Astaw Desta (License ID# 20160003864) for the premises located at 2140 Ford Parkway is hereby denied.
