Approving adverse action against the Solid Waste Hauler & Vehicle license application submitted by Curbside Waste, Inc.

WHEREAS, the Solid Waste Hauler & Vehicle license application submitted by Curbside Waste, Inc. d/b/a Curbside Waste, Inc. (License ID #20160003570) for the City of Saint Paul was the subject of a Notice of Intent to Deny License dated March 21, 2017, stating the licensee failed to submit required information to complete the application process; and
WHEREAS, the Department of Safety and Inspections made three (3) attempts within the past several months to contact the applicant in order to schedule a vehicle inspection and obtain the required application information; and
WHEREAS, the licensee did respond to the Notice of Intent to Deny License to schedule a vehicle inspection on April 4, 2017, but then failed to bring his vehicle in for an inspection; and
WHEREAS, the licensee also did not respond to the Notice of Intent to Deny License to submit the required information to continue the application process, request a public hearing or withdraw his license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to have his vehicle inspected and submit the required information, request a public hearing or withdraw his license application by March 31, 2017, that the matter would be placed on the next available consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Solid Waste Hauler & Vehicle license application submitted by Curbside Waste, Inc. d/b/a Curbside Waste, Inc. is hereby denied.
