Approving adverse action against the Cigarette/Tobacco license held by Richard A. Stai d/b/a 5th Street Convenience located at 55-5th Street East.

 

Body

WHEREAS, the Cigarette/Tobacco license held by Richard A. Stai d/b/a 5th Street Convenience (License ID #20050004334) for the premises located at 55-5th Street East in Saint Paul, was the subject of a Notice of Intent to Suspend License, stating the licensee failed to pay delinquent license and late fees; and

 

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information to maintain the license; and  

 

WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to to pay the delinquent license and late fees and submit required information or request a hearing; and 

 

WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information or request a hearing by February 14, 2017, the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the Cigarette/Tobacco license held by Richard A. Stai d/b/a 5th Street Convenience is hereby suspended for failure to pay delinquent license and late fees.

 

FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.

 

FINALLY RESOLVED, the license will remain suspended until all delinquent license fees and penalties are paid in full and all required information is submitted to the Department of Safety and Inspections.
