Approving adverse action against the House Sewer Contractor license held by Engineering & Construction Innovations.

 

Body

WHEREAS, the House Sewer Contractor license held by Engineering & Construction Innovations d/b/a Engineering & Construction Innovations (License ID #20120004674) for the City of Saint Paul was the subject of a Notice of Intent to Suspend License dated March 21, 2016, stating the licensee failed to pay delinquent license and late fees and submit required information; and

 

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information to maintain the license; and  

 

WHEREAS, the licensee responded to the Notice of Intent to Suspend License to inquire about payment of delinquent license and late fees and submission of the required information;

 

WHEREAS, the licensee then paid the delinquent fees and submitted most of the required information but failed to submit the required $10,000 Surety Bond; and 

 

WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit all required information or request a hearing by March 31, 2016, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the House Sewer Contractor license held by Engineering & Construction Innovations d/b/a Engineering & Construction Innovations is hereby suspend for failure to submit the $10,000 Surety Bond.

 

FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.

 

FINALLY RESOLVED, the license will remain suspended until the $10,000 Surety Bond is submitted to the Department of Safety and Inspections.
