Title

Approving adverse action against the Sidewalk Contractor license held by Clausen Construction, Inc. d/b/a Clausen Construction, Inc. for the City of Saint Paul.

 

Body

WHEREAS, the Sidewalk Contractor license held by Clausen Construction, Inc. d/b/a Clausen Construction, Inc. (License ID #20000001470) for the City of Saint Paul was the subject of a Notice of Intent to Suspend License dated June 16, 2015, stating the licensee failed to pay delinquent license and late fees; to submit Automotive and Non-Automotive liability insurance and a completed Certificate of Compliance Minnesota Workers’ Compensation Law form; and

 

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and  

 

WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees and submit required information or request a hearing; and 

 

WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees, submit the required information or request a hearing by June 26, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the Sidewalk Contractor license held by Clausen Construction, Inc. d/b/a Clausen Construction, Inc. is hereby suspended for failure to pay delinquent license and late fees; and

 

FURTHER RESOLVED, licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.

 

The license will remain suspended until all delinquent fees are paid and required information is submitted to the Department of Safety and Inspections.
