Approving adverse action against all licenses held by 961 Group, Inc., d/b/a Speedy Dale located at 589 Dale Street North.

WHEREAS, all licenses held by 961 Group, Inc. d/b/a Speedy Dale (License ID #20090002922) for the premises located at 589 Dale Street North in Saint Paul, were the subject of a Notice of Intent to Suspend Licenses dated January 8, 2015, stating the licensee failed to pay delinquent license, permit and late fees and submit required information to maintain the license; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to pay the delinquent license, permit and late fees and submit required information or request a hearing; and
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to pay the delinquent license, permit and late fees and submit the required information or request a hearing by January 19, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, all licenses held by 961 Group, Inc. d/b/a Speedy Dale are hereby suspended for failure to pay delinquent license and late fees.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.
The licenses will remain suspended until all license, permit fees and penalties are paid in full and all required information is submitted to the Department of Safety and Inspections.
