Approving adverse action against the Tanning Facility license held by CKF Holdings, Inc., d/b/a Perfect Tan at 373 Pederson Street, Unit #103.
Body
WHEREAS, the Tanning Facility license held by CKF Holdings, Inc. d/b/a Perfect Tan  (License ID #20120001125) for the premises located at 373 Pederson Street, Unit #103 in Saint Paul, was the subject of a Notice of Intent to Suspend License dated November 5, 2014, stating the licensee failed to pay delinquent license and late fees and submit the following information:  1) a list of tanning equipment including manufacturer's name, model #, type (booth, bed, canopy), year manufactured and number in the establishment and 2) a completed Certificate of Compliance Minnesota Workers' Compensation Law form; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and   
 
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees and submit required information or request a hearing; and  
 
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information or request a hearing by November 17, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Tanning Facility license held by CKF Holdings, Inc. d/b/a Perfect Tan is hereby suspended for failure to pay delinquent license and late fees.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.
 
The license will remain suspended until all license fees and penalties are paid in full and all required information is submitted to the Department of Safety and Inspections.
