Approving adverse action against the Taxicab Driver license application submitted by Diriba A. Tolasa.

WHEREAS, the Taxicab Driver license application submitted by Diriba A. Tolasa (License ID #20130004564) for the City of Saint Paul was the subject of a Notice of Intent to Deny License dated October 3, 2014, stating the licensee failed to provide: 1) a driving record under a former name of Diriba K. Gelete and 2) proof of a name change from the Minnesota Department of Vehicle Services; and
WHEREAS, based on this information, the licensing office recommended license denial and;
WHEREAS, licensee did not respond to the Notice of Intent to Deny License to submit the required information, request a hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit the required information, request a hearing or withdraw the license application by October 13, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Taxicab Driver license application submitted by Diriba A. Tolasa is hereby denied.
