Approving the issuance of a new Massage Practitioner license to Stephen Said Parker, and waiving the $500.00 matrix penalty previously imposed.

WHEREAS, Stephen Said Parker d/b/a Stephen Said Parker ("Applicant") has applied for a Massage Practitioner license (License ID #20140001151) for the premises located at 2140 Ford Parkway in Saint Paul; and

WHEREAS, the Massage Practitioner and Massage Practitioner-Additional Location licenses previously held by Applicant (previous License ID #20090004624) for the premises located at 150 Cesar Chavez Street in City of Saint Paul were the subject of a Notice of Intent to Suspend License dated March 29, 2011, stating the Applicant had failed to pay delinquent license and late fees of $175.00 and submit required information; and

WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and

WHEREAS, the Applicant did not respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees and submit required information or request a hearing; and

WHEREAS, a resolution was submitted to the Consent Agenda and on May 4, 2011, City Council approved the license suspension and imposition of the $500.00 matrix penalty (RES #11-810); and

WHEREAS, on February 28, 2014, Applicant contacted the Department of Safety and Inspections regarding reinstating his licenses and was informed that they were suspended and he would need to pay the $500.00 matrix penalty before he could obtain a new license; and

WHEREAS, the Applicant then informed DSI that, prior to the issuance of the above-referenced delinquency notices, he moved from his residence at 224 Cottage Avenue West, and left his employment at the 150 Cesar Chavez Street and 2375 University Avenue locations to take a job in Minneapolis, so he never received the notices, and was not working as ...
