Approving adverse action against the Sidewalk Contractor license held by Ground Support, Inc.

WHEREAS, the Sidewalk Contractor license held by Ground Support, Inc. d/b/a Ground Support, Inc. (License ID #20110004325) for the City of Saint Paul, was the subject of a Notice of Intent to Revoke License dated April 23, 2014, stating the licensee failed to submit current certificates of Automotive and Non-Automotive Public Liability insurance naming the City of Saint Paul as additional insured; and
WHEREAS, the licensee did not respond to the Notice of Intent to Revoke License to submit the required insurance information or request a hearing by the May 5, 2014 deadline; and
WHEREAS, the City Attorney's Office sent a letter to the licensee on May 14, 2014, stating that if he didn't respond to the letter by May 22, 2014, to submit the required insurance information that the matter would be placed on the consent agenda to revoke his license;
WHEREAS, the licensee failed to respond to the May 14, 2014, letter to submit the required insurance information; now, therefore, be it
RESOLVED, the Sidewalk Contractor license held by Ground Support, Inc. d/b/a Ground Support, Inc. is hereby revoked.
