Approving adverse action against the House Sewer Contractor license held by Blackstone Contractors, LLC.

WHEREAS, the House Sewer Contractor license held by Blackstone Contractors, LLC d/b/a Blackstone Contractors, LLC (License ID #20130003189) for the City of Saint Paul was the subject of a Notice of Intent to Revoke License dated March 27, 2014, stating the licensee failed to pay delinquent license and late fees of $196.00 and submit the following required documents: 1) current certificate of Public Liability insurance with a ten (10) day notice of cancellation and naming the City of Saint Paul as additional insured; 2) a $10,000 Surety Bond; 3) a list of the person/employee's name from their company who has taken the test with the Saint Paul Public Works Sewer Division; and 4) a completed Certificated of Compliance Minnesota Workers' Compensation Law form; and
WHEREAS, the licensee did not respond to the Notice of Intent to Revoke License to pay the delinquent license and late fee and submit required information, request a public or administrative hearing or notify DSI that they no longer wanted to operate in Saint Paul; and
WHEREAS, the Notice of Intent to Revoke Licenses stated that if the licensee failed to pay the delinquent license and late fees and submit required documents, request a public or administrative hearing or notify DSI that they no longer wanted to operate in Saint Paul by April 7, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the House Sewer Contractor license held by Blackstone Contractors, LLC d/b/a Blackstone Contractors, LLC for the City of Saint Paul is hereby revoked.
