WHEREAS, the Massage Practitioner license held by Anne C. Spiro (License ID #20120005538) for the premises located at 730 Cleveland Avenue South in Saint Paul, was the subject of a Notice of Intent to Revoke License dated February 27, 2014, stating the licensee failed to pay delinquent license and late fees of $135.00 and submit the following required documents: 1) current certificates of General and Professional Liability insurance with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured; 2) Proof of Affiliation from a City of Saint Paul licensed therapeutic massage center and 3) a copy of her Tax ID or Social Security Number.
WHEREAS, the licensee did not respond to the Notice of Intent to Revoke License to pay the delinquent license and late fees and submit required documents or request a public or administrative hearing; and
WHEREAS, the Notice of Intent to Revoke License stated that if the licensee failed to pay the delinquent license and late fees and submit required documents or request a public or administrative hearing by March 10, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license held by Anne C. Spiro for the premises located at 730 Cleveland Avenue South in Saint Paul is hereby revoked.
