WHEREAS, the Tree Trimmer & 1 Vehicle license held by Slibe A. Hodge d/b/a Slibe Hodge Tree Snow Authority (License ID #20130001801) for the City of Saint Paul, was the subject of a Notice of Intent to Revoke License dated February 27, 2014, stating the licensee failed to pay delinquent license and late fees of $214.00, schedule a vehicle inspection and submit the following required documents: 1) current vehicle information (year, make, model and license plate #); 2) a current certificate of General Liability insurance; 3) proof of registration with the Commissioner of Agriculture; 4) proof that you have employed an individual who possesses current certification as an arborist from the ISA and 5) a copy of their Tax ID or Social Security Number and 6) a completed Certificate of Compliance Minnesota Workers' Compensation Law form.
WHEREAS, the licensee did not respond to the Notice of Intent to Revoke License to pay the delinquent license and late fees, schedule a vehicle inspection and submit required documents or request a public or administrative hearing; and
WHEREAS, the Notice of Intent to Revoke License stated that if the licensee failed to pay the delinquent license and late fees, schedule a vehicle inspection and submit required documents or request a public or administrative hearing by March 10, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Tree Trimmer & 1 Vehicle license held by Slibe A. Hodge d/b/a Slibe Hodge Tree Snow Authority is hereby revoked.
