WHEREAS, the MAD-Game Machines (42 @ $17.00) and MAD-Music Machines (9 @ $17.00) licenses held by Thomas Suggs d/b/a Jubilee Music Company (License ID #0050567) for the City of Saint Paul, was the subject of a Notice of Intent to Revoke License dated February 28, 2014, stating the licensee failed to pay delinquent license and late fees of $1,215.00 and submit the following required documents: 1) list of the type of machine (snack, pool table, video game, etc) and location; and 2) a copy of their Tax ID or Social Security Number; and
WHEREAS, the licensee did not respond to the Notice of Intent to Revoke License to pay the delinquent license and late fees and submit required documents or request a public or administrative hearing; and
 
WHEREAS, the Notice of Intent to Revoke License stated that if the licensee failed to pay the delinquent license and late fees and submit required documents or request a public or administrative hearing by March 10, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the MAD-Game Machines (42 @ $17.00) and MAD-Music Machines (9 @ $17.00) licenses held by Thomas Suggs d/b/a Jubilee Music Company are hereby revoked.
