WHEREAS, the Massage Practitioner license held by Brightstar Harmon Inyang (License ID #20120003502) for the premises located at 2140 Ford Parkway in Saint Paul, was the subject of a Notice of Intent to Revoke License dated January 24, 2014, stating the licensee failed to pay delinquent license and late fees of $117.00 and submit proof of affiliation; and
WHEREAS, the licensee did not respond to the Notice of Intent to Revoke License to request a public or administrative hearing; and
WHEREAS, the Department of Safety and Inspections received an email from Massage Envy in Highland Village on December 30, 2013, stating the licensee was not currently employed or affiliated with that location; and
WHEREAS, the Notice of Intent to Revoke License stated that if the licensee failed to request a public or administrative hearing by February 3, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license held by Brightstar Harmon Inyang is hereby revoked.
