Approving adverse action against the Laundry/Dry-cleaning Plant license and Alarm Permit (Renew) held by Teng & Se Her, d/b/a Community Laundry at 490 University Avenue.
Body
WHEREAS, the Laundry/Dry-cleaning Plant license and Alarm Permit (Renew) held by Teng & Se Her d/b/a Community Laundry (License ID #20080002250) for the premises located at 490 University Avenue in Saint Paul, were the subject of a Notice of Intent to Suspend License dated December 18, 2013, stating the licensee failed to pay delinquent license/permit and late fees of $259.00 and submit a completed Certificate of Compliance Minnesota Workers' Compensation Law form; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and   
 
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the delinquent license/permit and late fees and submit required information or request a hearing; and  
 
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license/permit and late fees and submit the required information or request a hearing by December 30, 2013, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Laundry/Dry-cleaning Plant license and Alarm Permit (Renew) held by Teng & Se Her d/b/a Community Laundry are hereby suspended for failure to pay delinquent license/permit and late fees of $259.00.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.
 
The license and permit will remain suspended until all license/permit fees and penalties are paid in full and all required information is submitted to the Department of Safety and Inspections.
