WHEREAS, the Tree Trimmer & 1 Vehicle and Tree Trimmer - Each Add'l Vehicle license applications submitted by Michael R. Mickelson d/b/a Arbor & Home Specialist (License ID #20120003392) for the City of Saint Paul were the subject of a Notice of Intent to Deny Licenses dated December 18, 2013, stating the licensee failed to submit required information, schedule vehicle inspections and obtain the necessary approvals in order to continue processing the license application; and
WHEREAS, based on this information, the licensing office recommended license denial; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny Licenses to submit the required information, schedule vehicle inspections, obtain the necessary approvals, request a hearing or withdraw his license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit required information, schedule vehicle inspections, obtain the necessary approvals, request a hearing or withdraw his license application by December 30, 2013, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Tree Trimmer & 1 Vehicle and Tree Trimmer - Each Add'l Vehicle license applications submitted by Michael R. Mickelson d/b/a Arbor & Home Specialist are hereby denied.
