Body

WHEREAS, the Health/Sport Club license application submitted by Greenbody, LLC d/b/a Greenbody (License ID #20130003664) for the premises located at 328 - 9^th Street East, Ste. #1 in Saint Paul, was the subject of a Notice of Intent to Deny License dated November 6, 2013, stating the licensee failed to provide a detailed business plan and obtain the required inspection approvals and permits required to complete the application process; and

WHEREAS, the licensee responded to the Notice of Intent to Deny License by sending a letter to DSO to withdraw his license application; and

WHEREAS, the licensee was instructed by a DSI inspector in order to continue processing his license withdrawal request he would need to provide a signed statement that he intended to discontinue business operations at that location; and

WHEREAS, the licensee failed to follow-up with DSI to submit the signed statement by the November 22, 2013 deadline; now, therefore, be it

RESOLVED, the Health/Sport Club license application submitted by Greenbody, LLC d/b/a Greenbody is hereby denied.
