Approving adverse action against the Massage Center - A (Commercial) license held by Carol L. Jasin, d/b/a Tease Salon, located at 961 Seventh Street West.

WHEREAS, adverse action was taken against the Massage Center - A (Commercial) license held by Carol L. Jasin d/b/a Tease Salon (License ID #20090004373) for the premises located at 961 - 7th Street West in Saint Paul, by Notice of Intent to Suspend License dated March 22, 2013, alleging licensee failed to pay delinquent license and late fees of $333.00, submit a list of current practitioners with their license numbers and a completed Certificate of Compliance Minnesota Workers' Compensation form; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and
WHEREAS, the licensee did respond to the Notice of Intent to Suspend License on March 25, 2013 to state she is no longer offering massage at the salon and she will send a letter to DSI cancelling her license; and
WHEREAS, the licensee failed to follow-up after the telephone conversation to submit the letter to DSI cancelling her Massage Center - A (Commercial) license; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information or request a hearing by April 1, 2013, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Center - A (Commercial) license held by Carol L. Jasin d/b/a Tease Salon is hereby suspended for failure to pay delinquent license and late fees of $333.00.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.
