WHEREAS, adverse action was taken against the Food Vehicle, Food Vending Machine (6 machines) and Food Vending Machine Operator license applications submitted by A K Vending, LLC d/b/a A K Vending, LLC (License ID #20110002901) for the City of Saint Paul by Notice of Intent to Deny Licenses dated January 23, 2013, alleging the licensee failed to obtain the required Department of Safety and Inspections (DSI) Environmental Health inspection approval; and
WHEREAS, the licensing office recommended license denial; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny Licenses to contact DSI to schedule the required Environmental Health inspection, request a hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny Licenses stated that if the licensee failed to obtain the required Environmental Health inspection approval, request a hearing or withdraw the license application by February 4, 2013, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Food Vehicle, Food Vending Machine (6 machines) and Food Vending Machine Operator license applications submitted by A K Vending, LLC d/b/a A K Vending, LLC are hereby denied.
