WHEREAS, adverse action was taken against the Massage Practitioner license application submitted by Bao Sophie Lo (License ID #20120001026) for the premises located at 1001 Johnson Parkway, Ste. A15 in Saint Paul, by Notice of Intent to Deny License dated January 24, 2013, alleging licensee failed to submit Proof of Affiliation and either a notarized copy of current certification in Therapeutic Massage and Bodywork or successful completion of written and practical examinations from the City of Saint Paul's authorized examiner; and
WHEREAS, the licensee did not respond to the Notice of Deny License to submit the required information, request a hearing or withdraw her license application; and
WHEREAS, the Notice of Intent to Deny License stated if the licensee failed to submit the required information, request a hearing or withdraw her license application by February 4, 2013, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Massage Practitioner license application submitted by Bao Sophie Lo is hereby denied.
