WHEREAS, adverse action was taken against the Retail Food Butcher - Secondary Facility and Retail Food (B) Grocery 101-1000
sq. ft. licenses held by Udo's African Food Store, LLC d/b/a Udo's African Food Store, LLC (License ID #20090005072) for the premises located at 1459 University Avenue West in Saint Paul, by Notice of Intent to Suspend Licenses dated November 20, 2012, alleging licensee failed to pay delinquent license, permit and late fees of $304.00 and submit a completed Certificate of Compliance Minnesota Workers' Compensation Law form; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to pay the delinquent license, permit and late fees, submit the required information or request a hearing; and
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to pay the delinquent license, permit and late fees, submit the required information or request a hearing by November 30, 2012, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Retail Food Butcher - Secondary Facility and Retail Food (B) Grocery 101-1000 sq. ft. licenses held by Udo's African Food Store, LLC d/b/a Udo's African Food Store, LLC are hereby suspended for failure to pay delinquent license, permit and late fees of $304.00. Licenses will remain suspended until all license and late fees are paid in full to the Department of Safety and Inspections.
