WHEREAS, adverse action was taken against the Massage Practitioner license held by Aubrey J. Miller d/b/a Aubrey J. Miller (License ID #20100001383) for the premises located at 2301 Como Avenue, Ste. #102 in Saint Paul, by Notice of Intent to Suspend License dated August 22, 2012, alleging the licensee failed to submit a current certificate of General and Professional Liability insurance for the coverage period from August 31, 2011 through August 31, 2012 with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required insurance information in order to maintain the license; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to submit the required insurance information or request a hearing; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to submit the required insurance information or request a hearing by September 3, 2012, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license held by Aubrey J.Miller d/b/a Aubrey J. Miller is hereby suspended and will remain suspended until the licensee submits a current certificate of General and Professional Liability insurance for the coverage period from August 31, 2011 through August 31, 2012 with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required insurance information in order to maintain the license. Payment of such penalty shall be made within thirty (3...
