Approving adverse action against the Taxicab Driver license application submitted by Cody A. Fisher d/b/a Cody A. Fisher for the City of Saint Paul.

WHEREAS, adverse action was taken against the Taxicab Driver license application submitted by Cody A. Fisher d/b/a Cody A. Fisher (License ID #20120000984) for the City of Saint Paul by Notice of Intent to Deny License dated August 9, 2012, alleging the licensee failed to submit a copy of his driving record from the State of Utah as required by Saint Paul Legislative Code ยง 376.16 (e) (5) (e); and
WHEREAS, as a result, the licensing office recommended license denial; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny License to submit the requested information, request a hearing or withdraw his license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit the requested information, request a hearing or withdraw his license application by August 20, 2012, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Taxicab Driver license application submitted by Cody A. Fisher d/b/a Cody A. Fisher is hereby denied.
