Approving adverse action against the City Contractor-General Installation license held by Stoneman Masonry, Inc. d/b/a Stoneman Masonry, Inc. located at 749 Como Avenue in Saint Paul.

Body

WHEREAS, adverse action was taken against the City Contractor - General Installation license held by Stoneman Masonry, Inc. d/b/a Stoneman Masonry, Inc. (License ID #20060002386) for the City of Saint Paul, by Notice of Intent to Suspend License dated December 22, 2011, alleging after being sent a Second Request, the licensee failed to submit the following information: 1) a current certificate of General Liability insurance for the coverage period of January 23, 2011 through January 23, 2012 with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured and 2) a completed Certificate of Compliance Minnesota Workers' Compensation Law form; and

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and   

WHEREAS, On January 4, 2012, a contract employee for the licensee responded to the Notice of Intent to Suspend License to indicate that the insurance and workers' compensation information would be mailed to DSI that week; and

WHEREAS, On January 13, 2012, a fax was sent from the licensee with the requested insurance and Workers' Compensation information; and

WHEREAS, after inspection by DSI, it was found that the insurance information submitted had a lapse in coverage from January 23, 2011 through June 1, 2011; and

WHEREAS, on January 19, 2012, DSI sent a letter to the licensee requesting proof of coverage from January 23, 2011 through June 1, 2011; and

WHEREAS, the licensee did not respond to the letter sent by DSI requesting proof of coverage; and
