Body
WHEREAS, adverse action was taken against the City Contractor - Exterior Finishing license held by Final Touch Exteriors, Inc. d/b/a Final Touch Exteriors, Inc. (License ID #20060004077) for the City of Saint Paul by Notice of Intent to Suspend License dated September 19, 2011, alleging licensee failed to submit a current certificate of General Liability insurance and a $2,500.00 License Permit and Performance Bond; and   
 
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and   
 
WHEREAS, the licensee did respond to the Notice of Intent to Suspend License to submit a certificate of General Liability insurance on September 29, 2011, but failed to submit a $2,500.00 License Permit and Performance Bond; and
 
WHEREAS, a second request for the License Permit and Performance Bond was sent to licensee on October 14, 2011, and licensee did not respond, and
 
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to submit the required information or request a hearing, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
 
RESOLVED, that the City Contractor - Exterior Finishing license held by Final Touch Exteriors, Inc. d/b/a Final Touch Exteriors, Inc. is hereby suspended and the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.  Payment of such penalty shall be made within thirty (30) days of the date of the adoption of this resolution.
