Approving adverse action against the Wine On Sale, Malt On Sale (Strong) licenses and Alarm Permit (Renew) held by IPHO By Saigon, Inc. at 704 University Avenue West.

WHEREAS, the Wine On Sale, Malt On Sale (Strong) licenses and Alarm Permit (Renew) held by IPHO By Saigon, Inc. d/b/a IPHO By Saigon (License ID #20120000345) for the premises located at 704 University Avenue West in Saint Paul, were the subject of a Notice of Intent to Suspend Licenses & Permit dated February 24, 2015, stating the licensee failed to pay delinquent license and late fees and submit required information; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the licenses; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to pay the delinquent license and late fees and submit required information or request a hearing; and
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to pay the delinquent license and late fees and submit the required information or request a hearing by March 6, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Wine On Sale, Malt On Sale (Strong) licenses and Alarm Permit (Renew) held by IPHO By Saigon, Inc. d/b/a IPHO By Saigon are hereby suspended for failure to pay delinquent license and late fees.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the licenses.
The licenses and permit will remain suspended until all license fees and penalties are paid in full and all required information is submitted to the Department of Safety and Inspections.
