WHEREAS, the Sanitary Disposal Vehicle license held by Fostervold Transport Services, Inc. d/b/a Fostervold Trucking (License ID #00019689) for the City of Saint Paul, was the subject of a Notice of Intent to Revoke License dated March 13, 2014, stating the licensee failed to pay delinquent license and late fees of $828.00 and submit the following required documents: 1) current vehicle information (year, make, model and license plate #) and 2) a copy of their Tax ID or Social Security Number.
WHEREAS, the licensee did not respond to the Notice of Intent to Revoke License to pay the delinquent license and late fees and submit required documents, request a public or administrative hearing or notify DSI that they no longer wanted to operate in Saint Paul; and
WHEREAS, the Notice of Intent to Revoke License stated that if the licensee failed to pay the delinquent license and late fees and submit required documents, request a public or administrative hearing or notify DSI that he no longer wanted to operate in Saint Paul by March 24, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Sanitary Disposal Vehicle license held by Fostervold Transport Services, Inc. d/b/a Fostervold Trucking is hereby revoked.
