Approving adverse action against the Massage Practitioner license held by Patricia F. MacDonald d/b/a Patricia F. MacDonald for the premises located 1415 Almond Avenue in Saint Paul.

WHEREAS, adverse action was taken against the Massage Practitioner license held by Patricia F. MacDonald d/b/a Patricia F. MacDonald (License ID #20090003386) for the premises located at 1415 Almond Avenue in Saint Paul by Notice of Intent to Suspend License dated December 10, 2010, alleging licensee failed to submit current certificates of General and Professional insurance and Proof of Affiliation; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and
WHEREAS, the licensee did respond to the Notice of Intent to Suspend License on December 21, 2010 to indicate that she would send in the required information that day but failed do so; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to submit the required information by December 20, 2010, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Massage Practitioner license held by Patricia F. MacDonald is hereby suspended and the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license. Payment of such penalty shall be made within thirty (30) days of the date of the adoption of this resolution.
