Approving adverse action against the Food Vending Machine and Food Vending Machine Operator license applications submitted by The Vending Doctor, Inc. d/b/a Hospitality Vending for the City of Saint Paul.

WHEREAS, adverse action was taken against the Food Vending Machine and Food Vending Machine Operator license applications submitted by The Vending Doctor, Inc. d/b/a Hospitality Vending (License ID #20090002993) for the City of Saint Paul by Notice of Intent to Deny Licenses dated April 12, 2011, alleging licensee failed to contact DSI to schedule the required Environmental Health Inspection; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny Licenses to schedule the required Environmental Health Inspection; and
WHEREAS, the Notice of Intent to Deny Licenses stated that if the licensee failed to schedule the required Environmental Health Inspection by April 22, 2011, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Food Vending Machine and Food Vending Machine Operator license applications submitted by The Vending Doctor, Inc. d/b/a Hospitality Vending are hereby denied.
