Approving adverse action against the Recycling Collection Center license held by Vida Corporation, d/b/a Vida Corporation at 2327 Wycliff Street.
Body
WHEREAS, the Recycling Collection Center license held by Vida Corporation d/b/a Vida Corporation (License ID #20170001016) for the premises located at 2327 Wycliff Street in Saint Paul was the subject of a Notice of Intent to Suspend License dated October 5, 2018; and
WHEREAS, the Notice stated the licensee failed to pay delinquent license and late fees and submit a completed Certificate of Compliance Minnesota Workers’ Compensation Law form; and
WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office recommended a $500.00 matrix penalty for failure to submit information required to maintain the license; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees and submit the required information; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information by October 15, 2018 that the matter would be placed on the November 7, 2018 City Council consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Recycling Collection Center license held by Vida Corporation d/b/a Vida Corporation (License ID #20170001016) for the premises located at 2327 Wycliff Street is hereby suspended.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit the required information to maintain the license.
FINALLY RESOLVED, the license will remain suspended until the delinquent license and late fees are paid in full and all required information is submitted to the Department of Safety and Inspections.
