Title
Approving adverse action against all licenses held by Career Systems Development d/b/a Hubert H. Humphrey Job Corps Center located at 1480 Snelling Avenue North in Saint Paul.

WHEREAS, adverse action was taken against all licenses held by Career Systems Development d/b/a Hubert H. Humphrey Job Corps Center (License ID #20050001537) for the premises located at 1480 Snelling Avenue North in Saint Paul by Notice of Intent to Suspend Licenses dated December 16, 2010, alleging licensee failed to submit current vehicle information, a completed Certificate of Compliance Minnesota Workers Compensation Law form and call to schedule a Environmental Vehicle Inspection; and
 
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and
 
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to submit the required information or schedule a vehicle inspection; and
 
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to submit the required information and schedule a vehicle inspection by December 27, 2010, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
 
RESOLVED, that all licenses held by Career Systems Development d/b/a Hubert H. Humphrey Job Corps Center are hereby suspended and the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information and schedule a vehicle inspection. Payment of such penalty shall be made within thirty days of the date of the adoption of this resolution.
