Approving adverse action against the Tree Trimmer & 1 Vehicle and Tree Trimmer - Each Add'l Vehicle licenses held by S & S Tree Horticultural Specialist, Inc.

WHEREAS, the Tree Trimmer & 1 Vehicle and Tree Trimmer - Each Add'l Vehicle licenses held by S & S Tree Horticultural Specialist, Inc. d/b/a S & S Tree Horticultural Specialist, Inc. (License ID #0017938) for the City of Saint Paul was the subject of a Notice of Intent to Revoke Licenses dated February 27, 2014, stating the licensee failed to pay delinquent license and late fees of $482.00, schedule required vehicle inspections and submit the following required documents: 1) current vehicle information; 2) proof of registration with the Commissioner of Agriculture; 3) proof that they have employed a certified arborist with the ISA and 4) a copy of their Tax ID or Social Security Number; and
WHEREAS, the licensee responded to the Notice of Intent to Revoke Licenses on March 5, 2014 to pay the delinquent license and late fees and submit most of the required information, but failed to submit proof of ISA certification, current vehicle information and schedule the required vehicle inspections; and
WHEREAS, DSI sent the licensee a letter on March 20, 2014, to request current vehicle information and proof of ISA Certification;
WHEREAS, the licensee responded on March 28, 2014 to submit proof of ISA certification but still has not submitted current vehicle information and scheduled required vehicle inspections; and
WHEREAS, the Notice of Intent to Revoke Licenses stated that if the licensee failed to submit current vehicle information and schedule the required vehicle inspections, request a public or administrative hearing or notify DSI that they no longer wanted to operate in Saint Paul by March 10, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
