Approving adverse action against the Auto Repair Garage license held by Pet Enterprises Motortech, Inc., d/b/a Pet Auto Repair at 18 Acker Street East.
 
Body
WHEREAS, the Auto Repair Garage license held by Pet Enterprises Motortech, Inc. d/b/a Pet Auto Repair (License ID #201000004542) for the premises located at 18 Acker Street East in Saint Paul, was the subject of a Notice of Intent to Suspend License dated February 4, 2014, stating the licensee failed to pay delinquent license and late fees of $603.00, submit a Tax ID or Social Security number and a completed Certificate of Compliance Minnesota Workers' Compensation Law form; and
WHEREAS, per Saint Paul Legislative Code ยง310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and   
 
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees and submit required information or request a hearing; and  
 
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees and submit the required information or request a hearing by February 14, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Auto Repair Garage license held by Pet Enterprises Motortech, Inc. d/b/a Pet Auto Repair is hereby suspended for failure to pay delinquent license and late fees of $603.00.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.
 
The license will remain suspended until all license fees and penalties are paid in full and all required information is submitted to the Department of Safety and Inspections.
