WHEREAS, the Second Hand Dealer - Motor Vehicle license held by Corp Auto, Inc. d/b/a Corp Auto, Inc. (License ID #20030002015) for the premises located at 1221 Pierce Butler Route in Saint Paul, was the subject of a Notice of Intent to Suspend License dated August 25, 2015, stating the licensee failed to pay applicable fees and submit the following information: 1) completed applications forms with necessary documentation to DSI Zoning Division and 2) a completed State of Minnesota Department of Public Safety Driver and Vehicle Service Division Dealer Application form to DSI Licensing Division; and
WHEREAS, the licensee was also instructed to submit the following information to begin the review process in order to obtain DSI Zoning Division approval for an auto sales use at that location: 1) a completed Zoning Variance application form including all required supporting documentation and applicable fees requesting a variance from the development standards he could not comply with and 2) a completed Site Plan Review application form with all required supporting documentation along with applicable fee; and 3) a completed Certificate of Compliance Minnesota Workers’ Compensation Law form; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to pay the applicable fees and submit the required information or request a hearing; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay applicable fees and submit the required information or request a hearing by September 4, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
