WHEREAS, the Massage Practitioner license application submitted by Jennesse J. Perry (License ID #20140001563) for the premises located at 2140 Ford Parkway in Saint Paul was the subject of a Notice of Intent to Deny License dated July 31, 2015, stating that during a routine record check as part of the application process, DSI discovered that the applicant had a Felony Aggravated Assault case pending in Cobb County, Georgia; and
WHEREAS, as a result of the pending criminal case, the licensing office recommended license denial; and
WHEREAS, in the Notice of Intent to Deny License, DSI requested the applicant submit court documents from Cobb, County Georgia indicating the status of the Felony criminal case for their review before they could continue processing the application; and
WHEREAS, the applicant did not respond to the Notice of Intent to Deny License to submit the required information, request a public or administrative hearing or withdraw her license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit the required information, request a public or administrative hearing or withdraw her license application by August 10, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license application submitted by Jennesse J. Perry is hereby denied.
