WHEREAS, the Massage Practitioner license application submitted by Laura K. Sanchez (License ID# 20170002160) for the premises located at 1053 Grand Avenue Suite #113 in Saint Paul was the subject of a Notice of Intent to Deny License (“Notice”) dated October 27, 2017; and
WHEREAS, the Notice stated that the license application was still pending because the applicant failed to submit the following information: 1) payment of license fees with NSF Penalty Charge; 2) current certificates of general and professional liability insurance with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured and 3) proof of successful completion of a postsecondary course of study that includes 500 contact hours at an accredited or license school or successful completion of a national certification exam; and
WHEREAS, the applicant did not respond to the Notice of Intent to Deny License to submit the delinquent license fee payment and required information to complete the application process, request a public hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit the delinquent license fee payment and required information to complete the application process, request a public hearing or withdraw the license application by November 6, 2017, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license application submitted by Laura K. Sanchez (License ID# 20170002160) for the premises located at 1053 Grand Avenue, Suite #113 is hereby denied.
